SmilesByJag Referral Rewards – Terms & Conditions 

One patient. One referral card. One discount. One treatment plan.

1. Eligibility

• Valid for new patients only at SmilesByJag - Bricketwood Dental practice.

• Patient must be 18 years or over.

2. Single Use Policy

• Each referral card is valid for one-time use only.

• Only one referral discount may be applied per patient.

• The offer cannot be reused, duplicated, or transferred.

3. One Tier Per Treatment

• Patients may redeem only one tier discount (Tier A, Tier B, or Tier C).

• Discounts cannot be combined across multiple tiers.

• Example: A patient cannot receive £25 OFF and later return for £50 OFF under the same or a new referral.

4. Single Course of Treatment

• The referral discount applies to one course of treatment only, as defined in the initial treatment plan.

• The applicable tier is determined based on the full treatment plan agreed at the first consultation.

5. Upgrading Treatment

• If a patient upgrades to a higher-value treatment (e.g. Tier A → Tier B or C):

  • The higher-tier discount will apply instead as long it is within the same course of treatment plan

• Upgrades must be:

  • Within the same treatment plan

  • Agreed before treatment begins

• Discounts cannot be split across multiple appointments or future courses of treatment

6. Treatment Scope

• Valid on private dental treatments only at Bricketwood Dental Practice.

• Excludes:

  • NHS treatments

  • Hygienist services

• All treatment must be carried out by Dr. Jagdish Patel

7. Pricing & Payments

• Discounts apply to standard private treatment fees only.

• Discount is applied at the point of payment or treatment plan acceptance.

• No cash alternative or credit alternative is available.

8. Missed Appointments & Cancellations ❗

• If a patient:

  • Fails to attend (FTA), or

  • Cancels without sufficient notice
    The referral discount may be withdrawn at the clinician’s discretion

9. Refunds & Changes ❗ 

• If treatment is altered, reduced, or cancelled after starting:

  • The discount will be recalculated based on the final treatment provided

• Discounts are not transferable to future or alternative treatments

10. Misuse of Offer ❗

• SmilesByJag reserves the right to:

  • Refuse the discount

  • Withdraw the offer
    If misuse, duplication, or manipulation of the referral system is suspected

11. Time Limitation 

• Referral cards must be used within 6 months from issue.

12. Non-Combination Clause

• This offer cannot be used in conjunction with any other promotions, discounts, or finance offers unless explicitly stated.

13. Clinical Suitability ❗ 

• All treatments are subject to:

  • Clinical assessment

  • Suitability

• The dentist reserves the right to:

  • Decline or modify treatment if clinically inappropriate

14. General

• By using this referral offer, patients agree to these Terms & Conditions.

• SmilesByJag reserves the right to:

  • Amend or withdraw this promotion at any time without prior notice
